Public Submission Files

Public Submission is an add-on tool for RL6:Feedback. If your organization is using Public Submission to enable feedback submission, public users and other employees can access the web page and submit feedback to RL6 from their mobile devices or desktop computers. Once submitted, file managers can view and manage these submissions in RL6:Feedback.

Note: The ALLOW_PUBLIC_SUBMISSION general system parameter must be enabled in order for files to be submitted in RL6 using the Public submission form.

The public submission form consists of the following elements:

  1. Web form – The web form can display as an embedded form on an existing site or as a standalone web page, accessible from a URLClosedThe Uniform Resource Locator (URL) is the address of an internet resource, such as a web page. It includes the protocol used to access the page, such as HTTP. A sample URL is http://www.rlsolutions.com. created by your organization.
  2. Contact Details – This section provides text boxes allow users to enter contact information (last name, first name, phone, and email), a check box for users to indicate if they would like a follow-up communication, and a pick list field to let users identify themselves (e.g. Patient, Family of Patient, Anonymous)
  3. Feedback Details – This section provides fields to enter the subject and description of the feedback, select a date from a calendar if the feedback is related to an incident that occurred on a specific date, and select the location they are reporting on.
  4. CAPTCHA – CAPTCHA check box is used to verify the entry user is a person and not an automated bot attempting to spam the web application. (This field may be hidden if not used by your organization).
  5. Submit - when the form is completed users can submit their feedback. If there are validation errors (e.g. mandatory fields are missing information), a message is displayed at the top of the form and the field in question is highlighted. Once the required information is entered and submitted successfully, a confirmation page displays the Reference number (i.e. File ID) and is offered a link to start another submission.
  6. Clear - click this button to clear all fields on the form at any time (i.e. to start over or to enter a new feedback submission).
